Arizona's lien waiver law
Arizona sets four forms. A waiver and release is unenforceable unless it follows substantially one of them. The forms below are copied word for word from Arizona Revised Statutes section 33-1008, read on 2026-10-09. Blank lines in the statute are shown as underscores.
